Need linked blog removed from WP and Jetpack

  This site is deactivated but it is keeping me from being able to manage My Sites panel. When I try to go to My Sites, I get the pop-up to check Jetpack connection to primary blog. I am unable to remove it from my profile or control panel.

  The blog I need help with is: (visible only to logged in users)
