Need simple theme for pictures and survey.

  Having trouble finding a super simple theme for what I need. This website could be only 1 page and that would be fine.

  I need something that can display 3-5 pictures (slider would be fine)
  Right underneath that I need a survey to capture what picture the user likes best and their email.

  Would love suggestions for theme plus survey plugin. Thanks.
