Need to Remove Sign Up Box

I have put a sign up box on my blog and I wish to remove it. I am not sure how I put it on there in the first place. Any suggestions?

The site linked to your name is not a wordpress.com blog so we can’t provide support for it here. Self-hosted blogs are supported at wordpress.org. See the forum sticky on the differences b/w .com and .org if you’re unsure.

This is the site linked to your username, http://www.jimjdavidson.com/ . is NOT hosted here at wordpress.COM, and in fact it isn’t even a blog.
The blog you reference, which has no posts or any content, has a text widget in the sidebar to which you added the contact information. Go to appearance > widgets and remove it from the sidebar.
