Online risk assessment forms, what software to use?

    I am looking at creating an online risk assessment form and wondered if anyone had done something similar and if so could they recommend a plugin. Its important that anything I use will work as a PWA also.

    We do not have direct experience with risk assessment forms, but we do offer a form “block” you can use to create customized intake forms: https://en.support.wordpress.com/wordpress-editor/blocks/form-block/

    Is there some specific functionality that you need for our form such as the ability to upload documents or accept signatures? If so our built-in tools are not able to do that but you would have the option to upload any plugin or theme “designed for WordPress” and there are several form plugins to choose from for example: https://wordpress.com/plugins/?s=forms

    Please note that it is not possible to add plugins to free WordPress.com sites, but is possible by upgrading to our Business or eCommerce plans, both of which include this option:
