Opt-in form cannot installed

Hai, I have a problem to install the Opt-in form in my site.
I have already get the HTML code, pasted to the HTML area, the button DOWNLOAD is there. But, once saved, I go to the site, the form is not appear.
Any solution?
The blog I need help with is: (visible only to logged in users)

All that’s required to follow any blog hosted by wordpress.COM is exactly what you were required to provide to register here, and that is a valid email address that the published posts can be sent to.
Here subscribers control their own subscriptions and frequency of receipt of posts from sites they subscribe to. We cannot act on their behalf. More to the point is the fact that when email addresses are submitted for the purpose of receiving blog updates that is the only purpose they can be used for.

Widgets can be configured to appear only on certain pages (or be hidden on certain pages) by clicking on the Visibility button.

Hi, can you post the code you are trying to use here between code tags so we can have a look at it, like this.
<code> your form code </code>There are some limitations on the HTML and code that can be used here at WordPress.com for security reasons, and it could be that the code is not allowed.
We do have a MailChimp Widget, and some other services can be used, but would require you to use a standard link to the signup form on the service.
If you can paste the code here, and let me know which service you are wanting to use, I’ll see if there is anyway to make that happen for you.
