Order ID in Form

  So i am trying to create a form in the my accounts page, where users can fill in their details eg: name, address, passport number for an event.

  They will need to also pick which order ID correlates with the details, as 1 user may have more than 1 order, and the details are different.

  Any form that could do this would be helpful.

  It seems like you might be facing difficulties with a self-hosted WordPress.org website, and not with a WordPress.com-hosted one.
