Page template inquiry/advice

    I am trying to put together a nomination page for our charity efforts. I need some advice on which template might be best suited for the needs we have for the new program.

    WP.com: Yes
    Jetpack: No
    Correct account: Yes

    Further details: This is a nomination page. We will need the nominator to be able to write a paragraph explaining why the nominee is a worthy candidate and add a picture if they like. Then we need some type of LIKE button or Heart button for people to vote and indicate they like this one. The most LIKES or indicators will be chosen per annual quarter to receive the charity for that given program.
