Page that has harassment/someones personal information up.

I have come across a blog that has all of my personal information posted on it. This blog is not only displaying my personal information and residence to the public it is also harassing, slandering, and cyber bullying me. I need this page taken down ASAP, I’m honestly terrified at this point, ALL of my personal information is on it. I will not link the blog because of this. I need an Admin to contact me or direct me where I can have private contact with an Admin ASAP for the link to the blog to have it removed. This is extremely serious.
