paypal redirect problem

  I have a website called seolazy.com on this site are coded buttons that pay via paypal, the problem is i need it to redirect to a form after checkout but it does not redirect the user to the form, below is the code for the buttons please help and tell me if its something wrong with the code or another issue! thank you!

  That’s not a WordPress.COM site but a WordPress.ORG site, and you need to be over there for help with that version of the software.
