PDF form — fill out and sign

    I’ve tried to link to a PDF form from my site that can be filled out, signed and emailed. Once I add the PDF to my media library and linked it to my site, the e-sign and ability to fill out the form was no longer enabled. Signing up for EchoSign via Adobe, I received html code for an e-sign document: <script type=’text/javascript’ language=’JavaScript’ src=’https://secure.echosign.com/public/widget?f=6F4CWEXAXH6H4V’></script>
    but I have no idea how to add this code and have the appropriate form on my site.
