Just to clarify, is this the procedure you are following?
1. Visit https://wordpress.com/sites
2. Choose the site you want to disconnect.
3. Visit settings: https://wordpress.com/settings/general
4. Click on “Manage your connection” > “Disconnect from WordPress.com” at the far bottom, to remove this site from your WordPress.com sites list.If you ever get confused at a step, you can refer to this document. https://en.support.wordpress.com/disconnect-jetpack-sites/
If you are still facing issues/unable to disconnect this self-hosted WordPress (WordPress.org) site from your WordPress.com account, please wait for the WordPress.com Happiness Engineers’ response.
I see that the “modlook” tag is added to this forum topic. One of the WordPress.com Happiness Engineers should be able to help you through this.

In this case it doesn’t work. I can’t “Choose the site you want to disconnect”, because medialiteracy.net does not appear in my list of sites. But I’m still receiving Jetpack ‘Monitor’ notices for that domain.
