Please help migrate subscribers from old (self) host to new (self) host

I just moved my blog to a new host, and lost my email subscribers (via jetpack). Can you help?

Do you need the old/new IP’s ?

Old IP : http://69.89.31.156/
New IP : http://198.101.164.57

The site has already migrated to the new host.

the new site is

http://beardandbonnet.zippysites.com

Can I get my stats from the old site to pull through as well?

I have moved over all your WordPress.com subscribers as requested, from beardandbonnet.wordpress.com to http://www.beardandbonnet.com
