Please help, what am I doing wrong? SEO friendly pages

Can I please get some help?
When I created the wordpress account, the original permalink structure was
mydomain.com/?p=123
The wordpress file is titled index so I have the redirect set that typing mydomain.com will automatically go to mydomain.com/index
To make it more SEO friendly, I tried to change the permalink structure so that its
mydomain.com/index/%category%/%postname%/
and I tried
mydomain.com/%category%/%postname%/
Both of those result in the same thing. Clicking on any of the links in the menu bar gets a 404 page not found result.
I’ve even tried saving all my information, and deleting all the pages, then redoing them. It didn’t help.
What am I doing wrong? Should I remove the redirect?
