I removed qifamily.org from your account, so you’re all set with that.
You can remove yourself from WordPress.com sites (such as mftarttherapist.wordpress.com) from your My Blogs page. There, you can hover over the blog you want to leave and click the “Leave Blog” link.
Please let me know if I can do anything more to help with that. :)

@webgurumasters — I can remove those for you, but I want to warn you about sites that are still running but managed by other people now. If I remove those sites from your account, that will also disconnect Jetpack and disable any Jetpack features on those sites.
For sites that are still using Jetpack, the best way to remove them is to go into the Jetpack section of the dashboard, disconnect Jetpack from WordPress.com, and have the current admin reconnect Jetpack to their WordPress.com account.
If you’d prefer, I can go ahead and disconnect those sites from your account now, but be aware that any features that rely on Jetpack on those sites (including things like stats) will stop working.
