  Hi everybody, I would like to know if any of you know a plugin to create this kind of dynamic contact form.
  This is the website that is using this kind of form I want to implement in my website.

  Since using plugins here requires a paid Business Plan, you’re not likely to find many folks who can offer a recommendation.
