Plugin to distribute content by email???

    We are producing a pdf report for every state on something having to do with banks in that state and we want to be able to automatically distribute the report pdf by email based on the state the user selects. More specifically, I want the user to be able to enter their email address, their name, check a couple of boxes and then select the state they want a report on from a selection widget and hit submit. I would like the system to then log their info in a database and send them a copy of the specific report by email. I want to send it by email so that I am sure I have their address.

    Does anyone know of a plugin that can do this?
