Plugins for Freight forwarding

Hi there I am looking for a plugins on what our customers can send us quotation requests for their shipments
In this we need to customers address
Items descriptions. When they enter number of pcs automatically that number of lines need to be open to enter the dims of each box or bags. Each line must have a box to enter estimated weights and each line of dims also need a box of volume weight calculator.
At the bottom there need to be two boxes one shows total of actual weight and other shows total volume weight total.
Each inquiries must raise a unique requests reference number as we are planning to use it on different websites and reference number must be created according to that website so we can know from which website this request has been come to us.
I will do appreciate if you could help me with this.
