Plugins for Live Chat

Hello,
I have a health/wellness site, and would like to set it up so that I can do live chat consultations. I know there are many plugins you can use for live chat, but they all put a live chat window in the footer. I would really like to have the customer pay for the consultation, and then access a live chat session at the time of their appointment. I was thinking maybe they would get a link to open the live chat session in their email. Do you have any suggestions for how to set this up?
