Pop up opt in form with clickable bullet points

  I am trying to create a pop up opt in form that has bulletpoints where the client can click which service they need. I am trying to create one that looks similar to this one: http://jpvestidas.daportfolio.com/gallery/89892#2. The SEGOLY site that has the pop up on the side of the page with the service that the client can click for what they want. Does anyone know how to create a pop up similar to this?

  I’m sorry but pop-ups like that one cannot be used on a WordPress.com blog. What can be used is this customizable [contact-form] http://en.support.wordpress.com/contact-form/

  Can’t do it at wordpress.com since we can’t use script for forms HTML for security reasons. You have two choices:

  1. Use the customizable contact form wordpress provides (won’t be a pop up though).

  2. See if you can create a form at Wufoo that will do what you want (won’t be a pop up though). Wufoo forms are now supported here.

  Ok thanks for the help, I already have a contact page created using the Easy Contact Forms Plugin. I know that Aweber is a good site to create Pop-ups on. I will use that.
