  Is it possible to add posts to different pages, or do new posts just always show up on the ‘home’ page?

  You can’t add posts to a page, but since posts automatically appear on their category/tag pages, you can link to those in your menus.

  For example
  http://80dayzofcollege.wordpress.com/tag/college/ will show all your posts tagged “college”

  You can link to that page using a menu:
