Posts don’t save, misattributed revisions

Hi all,
I’m having some problems with a WordPress server and I’m hoping to get some help. Last weekend I edited a bunch of posts, and got the “Post saved” message after I hit “Update post” – but the posts never actually saved. (Going back and doing ti again seemed to help.)
Also, on one post in particular, a very strange Revision History issue occurred – I made some changes to a post, and then the next day several revisions appeared to it undoing those revisions. They were attributed to the admin user, but that person did not make these changes. (Also, a change I made was attributed to another user, even though I made/saved it.)
We are not using the latest version of WordPress, which may be an issue – but has anyone else ever encountered and resolved issues like this?
