Premium customer can't access direct e-mail support: "Contact Support form"?

I am a premium customer and I would like to ask some questions through direct e-mail support. However, I get nowhere when I click on the link that should take me to the “Contact Support” form — it’s the top item in the list, on this page:
http://en.support.wordpress.com/help-support-options/There is nothing about a Contact Support form on the page that is linked to. Please direct me to the place where I can access “direct e-mail support.” Thank you! :)
The blog I need help with is: (visible only to logged in users)

You must be logged in as Admin http://en.support.wordpress.com/user-roles/#administrator under the exact same username account that registered the blog. This is the correct link for the support contact form http://en.support.wordpress.com/contact/ which will be emailed to Staff.

I am absolutely certain that I am logged in as Admin and as the purchaser of the blog’s upgrades — I recently paid for renewal when logged in as this username, and I can view it in “Store” — but the page you link to shows me no way to write an e-mail.
Is it possible to tell me if this username is indeed *not* the username that originally registered the blog? If I am logged in as the original username, will an e-mail contact form show on the page you link to? Again, thank you!

The form @TT linked to jumps you through a pile of hoops before anything can be sent to the staff the last time I used the form – they really really don’t make it easy to send in a direct request for help – so the form is confusing at best and when I sent something direct to the staff I was never quite sure how things worked since it was literally the last step to send to task – they want you to use the forum –

Thanks, auxclass! Your comment made me realize that the form is “under” there but first I have to enter some words in the search box and then click down below to say that I still need more help than the search results.
