  If I get upgraded to Premium, would I still have to pay for a Premium theme? In effect, after paying the $99 would I still need to pay the $75?

  Yes you have to purchase it separately. The premium upgrade $99. does not include premium themes as you can clearly see if you read this page. http://store.wordpress.com/bundles/
