If I make a change to the Discussion Options page and
1) Check the following:
“Comment author must fill out name and e-mail”
“Comment author must have a previously approved comment”2) Uncheck the following:
“An administrator must always approve the comment”3) Uncheck the “Users must be registered and logged in to comment” on the general page.
What will trigger the comment author being recognized the next time around after the first moderated comment is approved by me? email or name or both? Am I opening the door to wide if I do this regarding spam?

Should be checking the following:
-Comment author must have a previously approved comment
-An administrator must always approve the comment#2 sends it to approval but #1 overrides that I would think.
#3 should be checked to prevent spam but that may annoy your visitors since they may or maynot have a wp.com account.

Ok I have not exhausted the testing, but found some results. For what it’s worth the testing was done on FireFox 2.0.0.3 and with the theme Rubric 1.0. Not sure if this will help anyone but here goes:
If I have the settings above in the original post then every comment I make (by those not signed into wp.com) will be held for moderation. The user is forced to put in a name and email like the checkbox says. If I then approve the comment, then the commentor will be able to comment again ONLY IF both the name and email address are identical to the first comment that was approved. Any new name with the same email is held for moderation.
If I then delete all of the approved comments made by this user, and they then try a new comment with the same name and email then they are held for moderation again as if they never commented before or were approved and the process begins again (sounds obvious enough, but figured I’d state it).

The above post holds true as well even for wp.com users that are signed in who are not associated with the blog. Obviously any admin of the blog can leave comments without approval.
