The WordPress.com Premium upgrade that you’ve purchased includes Custom Design, which allows you to use Custom Fonts, Custom Colors, and Custom CSS. It does not include premium themes.
With premium themes, you’ll need to purchase each theme separately — this is a one-time purchase, and you’ll be able to use the theme for as long as you wish. Alternatively, the Unlimited Premium Themes upgrade costs $120 per year, and gives you access to all our premium themes, so you can experiment and change themes as often as you want.

Thank you, Ben, but I remain confused and disappointed.
Here’s the marketing language I keyed on when I dropped my 30 bucks on “Custom Design:”
“With the Custom Design upgrade you can make your blog look and feel exactly the way you want. Customize the fonts in your theme with ease, apply a custom color palette and background pattern, or dive into CSS to make all the presentational changes you desire.”
I don’t find what I should have found there, a giant caveat that says I’ve got no real ability to customize. In fact, this language is very misleading, as I am apparently not going to be able to make my blog “look and feel exactly the way I want,” nor will we get to “make all the presentational changes we desire.”
I’m working with a firm that put together a pretty simple website for me, but they designed it themselves. So are you telling me now that even if I pay $120, my website still has to conform to whatever limited number of themes you folks dreamed up?
I feel misled, to put it bluntly, but I very much hope I’m either misunderstanding, or I might be eligible for a full refund of my $30.

There are two primary ways of customizing a WordPress.com site. The first one is with the Theme that you select. The theme largely dictates the look and feel of the site. We offer many of free and premium themes to pick from.
WordPress.com themes come with lots of customization options. Many themes allow you to set a custom header image and custom background, modify your color scheme, and much more. Go to Appearance → Themes in your Dashboard to view your theme’s customization options.
The second way, is the Custom Design upgrade. This allows you to change the CSS and fonts on your site, within the structural limits of the theme you have selected. If you know CSS you can do this yourself, or request theme customization from a 3rd party developer as you have mentioned.
