Problem with adding a form to my blog

I wanted people to be able to contact me, but didn’t want people to know my real email address, so I went into my “About me” page and clicked on the “Add Contact Form” box to add a contact form to my blog. Then I clicked “Update”.
When I went to view the actual page, I saw that my real email address and blog name had been entered into the form automatically. Obviously I do not want strangers to know what my real email address is, so how can I rectify this problem?
I have tried deleting the form and adding a new one again, but still the same thing happened. My real email address is there for all to see on the contact form on my page!
I have since removed the contact form from my page, and now I’m hoping someone here can help me solve this problem.

Put the form back; this is a common mistake. You don’t need to worry.
