Problem with CP Contact Form

  I need help with my CP Contact Form, located http://www.awarding.ca/contact

  As you may notice, the text is formatted all wrong. If I use the Preformatted code, the contact form text is no longer jumbled up to the left, however, it becomes way too long, and has other issues…

  Please help!!
