Problem with Custom Form

I have created a Custom form in my Contact Page.
When you open the page… the ‘name field’ includes my display name and the ’email field’ displays my email. I would like all fields to appear blank.
How can I resolve this issue? -
If you log out, clear your browser cache and cookies, restart your browser and take a look, the fields will be blank. WordPress.com uses cookies to identify returning visitors and will fill in that information for them when they return. Does the same thing with the comments forms. Your name and email would be filled in when you visit a form here. Someone else would see their name and email.

I actually just fixed that myself! But dont know if my fix caused my next problem. What i did was put a space after “name” in the code. It looks like this “name ” and then did the same to the email category. Note: i did it to the name and email that are not capitalized. When i saved and previewed all fields were blank.
However, I have not gone live yet with my blog – so when i tested my contact form and hit submit it comes into my email with “<> add contact” as the first line. Is it suppose to be blank??
All of the other information shows up fine.

Your welcome! But here is the problem… LOL…
As i asked about it in my other post … it is true, when i make that change and someone sends a note the first line is blank in the email. This means i dont know who sent it until the note is opened. So if i need to refer back to that email i will need to do a search… (the name appears inside the note) i even looked at the feedback page… same thing.
So i guess i have to decide how i want the contact form displayed!

This isn’t necessary and as you have found out, it breaks things. Read my description above of how the form works above. Your name and email will only show because wordpress knows who you are. No one else will see your name and email address. If it is their first time to your site, the fields will be empty. If they are a returning visitor, and have not cleared their cookies since their last visit, wordpress will fill in their name and email as a convenience for them. If they have cleared their cookies since their last visit then the fields will again be empty.
