Hello,
I would like to report that Previews of Uploaded Header Images in the Header section are not showing up for me, the Random option in the Header section is not working (if I select the Random option the Header will get bigger like a header image is there but none of my images show up so it is just the default reddish colored Header area in the Goran theme but bigger), and Previews of images in the Media Library are not showing up for me either:

I have tested this on Ubuntu 14.04 LTS with the latest Firefox in Safe Mode (and the last two versions of Firefox) and in Normal Mode, and in the latest version of Google Chrome; and with the Goran and Big Brother themes.
In Google Chrome I see what I think are broken image icons for the previews for my images in the Media Library and in the Header section, like all of my images are possibly broken oddly even though I tried adding new ones today; and that is probably why the Random option is not working.
Maybe something is wrong with my WordPress.com Account?

We were having an issue with images a few days ago. Are you still having this problem with your Media Library and the Header selector? Do you have the same issue with the Header images when you view them in the Customizer?

All of the problems are still there for me.
Under the Header Section under Uploaded Images, none of the previews/thumbnails of images are showing up at all like they used to:

In the Media Library previews/thumbnails of images are still not showing up like they used to, I only see gray boxes:

Under the Header Section the Random option is still not working.
I have been having these problems for weeks/maybe a month/maybe over a month now.
Thanks to you I now also know that header images are not showing up at all in the Customizer, when I am in the Customizer no header images are listed at all, even though I have uploaded header images; and so that is a fourth problem that I am having.
This is strange.

I am also still randomly having the grammar/spell check problem in the Classic Editor (https://en.forums.wordpress.com/topic/proofreading-suggestions-not-showing-up-on-the-first-try-most-of-the-time?replies=24#post-2025915), which seems to only be effecting/affecting me since no one else seems to be able to reproduce this except for me at this time, and so I am starting to wonder if something could be wrong with my account and/or something like that because of the unusual history of my account which goes something like (there is more about my account history that is not listed below):
My account history is a bit unusual because originally it was a Windows Live Spaces blog that was moved to WordPress.com when Microsoft discontinued Windows Live Spaces, eventually I created a new account and imported my old one to it, I created another new account and I imported the previous blog to it, I changed my username, and then I deleted my two oldest blogs leaving me with only one now; and so maybe there could be something strange happening where certain things are still linking to one or more of the deleted accounts and/or my old username?

Thanks for reminding me! I just updated that thread. :)
I just took a look around your Media Library and none of them are loading for me either. The log for your site shows that you deleted all of those files. We did have an issue with image caching this weekend, which might be why you’re seeing ‘ghosts’ of those images in your Media Library. You can remove these ‘ghosts’ by selecting ‘Delete permanently’ or let me know and I can do it for you.
Changing your username and deleting sites shouldn’t affect your account overall. And the transfer from Windows Live Spaces shouldn’t cause this either. You had mentioned in our previous thread that in Safe Mode the proofreader worked okay. Is this still true? Or did you start seeing this issue in Safe Mode as well?

I deleted all of the images in my Media Library, then I uploaded two new header images, but thumbnails/previews for the new images are still not working in the Header section or in the Media Library and the Random option for Header images is still not working (so you can only have one header image set to show); and so the problems are still there for me. :(
About my account, I did not think so, but I left it open as a small possibility; thank you for answering that. ;)
I am not sure if Proofreading works in Safe Mode all the time or not because I only tried it a few times, I would need to use it all day for at least three days to be sure, but I need some of my extensions every day so I have not tested this yet because I would not be able to do certain things online each day if I only used Safe Mode for several days; this problem is very random so I am not sure, I just know that in the few times that I tried Safe Mode (which was not enough to be sure) the problem did not happen, but it might happen if I use Safe Mode long enough because this problem is so random.

Hmm. Thanks for checking that out. I just tested uploading an image to your Media Library and seems to have worked correctly. Can you visit your Media Library and let me know if you see the image of a flower that I uploaded?
When you upload Header images, you’re doing that through Appearance > Header? Or the Customizer? Let me know and I’ll do some testing.
It looks like something is wrong with the uploading of your images. Can you upload images from these four places and let me know what the results are? (Maybe title the images differently so you can see which are which once they’re uploaded. :)
- Media Library > Add New
- Appearance > Header
- Customizer > Header
- Post > Add New
Looking forward to the results so we can get this sorted out for you!
I’m not sure what to test further with the proofreading. I’ve still been unable to recreate the issue on my computer. And it still sounds like its browser specific. Do you have Chrome on your computer? If not, it would be interesting to know if this happened with a clean install of Chrome.

Yes, I see the flower image that you uploaded to my Media Library.
When I upload header images I use the Dashboard | Appearance | Header | Select Image | Browse | Upload | I crop the image using the default size that the crop box/whatever is set to.
Media Library:
The thumbnail/preview worked when I uploaded it directly to the Media Library.
Appearance:
The thumbnail/preview did not work in the Header section or the Medial Library when I uploaded the image from Appearance | Header | Select Image | Browse | Upload.
Customizer:
The thumbnail/preview did work in the Customizer, the Media Library, and in the Header Section when I uploaded the image from the Customizer.
Post:
The thumbnail/preview did work in the Media Library when I uploaded the image from a Post in the Classic Editor (but the New Editor would not load for me today oddly, I do not really use it, but I usually have to let it load to switch to the Classic Editor; but today it was stuck at the Beep Beep Boop screen even after Refreshing twice, and so I had to manually go into the dashboard to add the image to a Post).
I do have Google Chrome, only for rare testing purposes, and so I have only tested it a few times without problems; but not enough to be sure.
The Proofreading problem is not a huge problem but I would like to solve it out of curiosity (but I feel like it is probably something that an expert will have to solve exactly, even if we do figure out it is only a Firefox related issue, an expert will have to figure out why) , but I just do not feel having to use Safe Mode or Google Chrome for several days at this time; but I might try using Google Chrome for several days maybe one day, if I get enough inspiration, who knows. ;)

Thanks so much for doing all of that John Jr! I was able to recreate the image issue on my test site when I upload to Appearance > Header. I’ve reported it to our developers and I’ll let you know when I hear back from them.
For now, if you upload your Header image through the Customizer you should be okay and you won’t see these broken images.
As for the proofreading, let me know what you find when you get around to using Chrome or Safe Mode for a while. Then we can help you further. :)
