Problems with Text Widget

  I am trying to copy code from ask.fm to a text widget to display a box where my readers can submit questions anonymously, but whenever I hit save, the code disappears and there is nothing on my actual page, other than the title of the widget.

  Also, is there anyway I can use the same code to create a widget on my actual contact page, or just on the sidebar with all the other widgets I use?
