Hi. If I purchase a theme, do I have to upgrade my wordpress.com service to access the customization options? Right now, I’m using the free service, but am looking at purchasing a theme to update my site.

Yes. Purchasing a premium theme is a separate purchase for a single blog that is a one time payment for the lifetime of the blog while hosted by WordPress.COM. http://en.support.wordpress.com/themes/premium-themes/
The custom design upgrade is a separate purchase that applies to a single blog for a single year only when it is due to be renewed.

Please let me add this as bloggers can become confused. The only themes that can be used on any WordPress.COM blogs are found on your Dashboard at > Appearance > Themes and are showcased here http://wordpress.com/themes/
There is no FTP access to free hosted WordPress.com blogs for security reasons and we cannot:
upload any third party themes,
create child themes,
create our own themes,
edit templates,
create own own layouts,
or use stylesheets from other themes.Relevant support docs:

http://en.support.wordpress.com/themes/adding-new-themes/You cannot upload any third party themes into a WordPress.COM hosted blog so be clear on that. There is no upgrade you can purchase here that changes that reality.
WordPress.org offers free software that you can install on a web server. You can create themes, upload and install themes and plugins, run ads, and edit the database, etc. If you want FTP access and to install plugins or third party themes into a site then you will have to hire a web host http://wordpress.org/hosting/ and set up your own WordPress.org install.

I understand the difference between wordpress.org and wordpress.com…what I’ve noticed is that for the free templates (here at .com), you have to upgrade your service to access the customization. Do I have to do that with a template I purchase here at .com? Or will I have access to the full customization which comes with the template because I bought it?

If you buy a premium theme, that is still completely separate from the customization option. They are two separate purchases. Voice of experience here.
