radio button & checkbox

Hi guys,
I tried to creat forms with radio buttons and checkboxes. However, I cannot select radio buttons or checkboxes after publishing the form. I tried multiple plugins to create the form and also tried to add CSS snippet but nothing helped. Could you please help to figure out what is wrong? Thanks a lot! The test form is here.
