random header suddenly showing up

i’ve been logged out of my admin account and can’t access my site. Besides this, the site is also prompting visitors for passcodes and showing random information at the top of the main page.
A timely response would be very much appreciated as this is my business website and this certainly does not look professional!

It absolutely is a wordpress site. I use the wp-admin to login (or at least I used to) and I just received a message from word press saying Truth BE Told has automatically been updated to version 4.0.3.
I believe this update is what has corrupted the code and is not allowing me access. I would send a screen shot but I’m not sure how to upload a jpg here.

Sorry for the confusion! It looks like http://truthbetoldcreative.com/ is using WordPress software, but it is not hosted here at WordPress.com. Because of that, http://wordpress.org/support/ is the right place to ask in your case.
Another thing you can do is contact your web host directly to see if they can help you learn how to check your site’s error logs. If you can find any, they might help get you to a resolution faster. You should include any errors you find or any other details you can think of in a new help request at http://wordpress.org/support/

I checked http://truthbetoldcreative.com/ again just now and I don’t see the form that was there before. Your site looks great btw!
