Random items being added to orders that customer didn’t pay for?

I have experienced 3 times now with 2 customers my website just adds random products to customers orders, they arent charged for the items only what they ordered. It changes the quantity to all the products added and the error also shows up in the order tab.
–for example–
Customer ordered 2 €10 room sprays and shipping was €7.
The customer was charged €27
But her order confirmation email and my order on the website showed
-room spray
-room spray
-gift box
-oil burner x2
-waxmelts
showing the Total €101 in her email and on my order tab, in my sales over view, and counted the add on products towards my items sold?This occured again twice to another customer where her order was duplicated. Again order was €73, total said €146 but she was only charged the €73
Does anyone know why this is happening? It’s causing my customers stress having to check their bank statements and messing with my statistics
