Re Create a Contact Form. My email and co name are pre-filled

Hi, When I create a Contact Form a Page, it shows up with my own contact information in it. When I click Publish, the Page shows up with MY company name in Company Name field and MY personal email in Email field. They should be blank, because the contact form is for visitors to my blog. I have searched help and have not found any answer. Can you help? Thanks

The blog I need help with is: (visible only to logged in users)

Not to worry. There is no error. WordPress.com uses cookies to keep track of commenters so if someone makes a comment on your blog, the next time they come to your blog, unless they have cleared their cookies, WordPress.com will recognize them and complete the details for them. No one else will be able to view those details in comment boxes or in contact forms. The next person who comments will see blank fields. Commenters will not see each others details.

While we are at it the same can be said about the Admin [edit] link. It’s only for your convenience and only you can see it and use it while you are logged in.

I should have included on more sentence above.
Not to worry. There is no error. Log out, clear your browser cache and cookies http://www.google.com/support/accounts/bin/answer.py?hl=en&answer=32050 and view your contact page like you are a visitor.

I will believe you and go Publish fearlessly, even with my private email address staring out of that field at me ;-) Thanks for your help.
