How do I create a site with the name I already created.
The blog I need help with is: (visible only to logged in users)

By old name do you mean address like bob.wordpress.com ? If it is the case, you cannot claim back the address as stated here : http://en.support.wordpress.com/delete-site/ Why ? It is explained here : http://en.support.wordpress.com/recycling-blog-names/
If you want the name of the site to be something in particular, you can change it in Settings->General in the Dashboard.
