What do I need to do to request a full refund that includes the approx. $13 ‘mapping’ charge. I upgraded and immediately cancelled (within 5minutes of upgrading) so the mapping was never used.

I’ve just refunded $13 to you for domain mapping — the plan has now been refunded in full.
Refunds usually take about a week to process and show up on your statement. You can view, print or email yourself a copy of the refund receipt from your Billing History here:

I received this mail from WP after cancellation of my wordpress premium upgrade on April 15 2017. But the money is not yet credited in my account.
Please do the needful.The following upgrade has been cancelled and $73.00 refunded:
