Remove a Site

  I have a self-hosted wordpress site that I manage http://dragonflypaddlers.com/. I’ve discovered there’s a WordPress.com site called https://shioctonkayakcanoe.wordpress.com which is using our company info and pictures but was not created or authorized by us. This site has our phone number, and address but all the pricing is incorrect and customers are finding this info and expect to receive this pricing which they found on this fake site. How do we get this site removed?
