remove autopopulated personal contact info on contact page

  I just built my website and included a contact section. Each time I visit the page it autopopulates my personal information. How do I remove this and have a blank contact section?

  The blog I need help with is: (visible only to logged in users)

  Others visiting your contact form will not see your information. They will see a blank form UNLESS they are logged into WordPress.com. If they are logged into WordPress.com, they will see their own information pulled into the form. Otherwise they will only see a blank form.

  For example, since I am logged into WordPress, when I go to your form, my information is filled in.
