Remove information from blog contact form

  Good morning,

  How do I remove my information from the contact form on my page, as I assume it is to be filled out by those wishing to contact me?

  The blog I need help with is: (visible only to logged in users)

  Hi there,

  The contact form autofills the info of the user viewing the page. So if you visit the contact page by logging out or in a private/incognito mode, you’ll see the form empty.
