Remove my blog name and email from contact form

  I have a page with a contact form and under the name section my blog name appears, and where the person is supposed to type their email – my email address appears. How do I change it so that they will be blank?

  The blog I need help with is: (visible only to logged in users)

  Hi Talesandtrends!

  There is an auto fill feature for forms. Your info is showing up because you are logged into your WordPress account. Your name and email address do not show up for other people.

  If you’d like to test it, you can log out of your account and then visit the contact page of your website.
