remove/add a page

  Hi ,
  I had a page as Contact by default on my theme, I add a contact form plugin, when I wanted to add my address and link it to google map there was 2 problems
  1: I added the address and linked it to google map , it appears in my original website in double !!! I have no idea what should I do
  2: I removed the Contact page and add another one , install the Form plugin again, I cannot see it in my Contact page, what did I do wrong?
