Request Feedback Form/Domain Fee

  The request feedback form is gone from my post. How can I restore the request feedback form on my post so I can send the request?

  Also, I receive a letter in the (USPS) mail what appears as a bill but states it is a solicitation, from Web Listings Inc.for $85.00 (Description of Services, Annual Website Search Engine Listing). I believe my fee for domain is paid through WordPress but wanted to make WordPress aware of this solication. Claudia

  Do you mean this Request Feedback feature? What do you see on the Edit Post form where the Request Feedback form should be?
