  The Request Feedback widget on my blog seems to have disappeared. How can I use that function?

  There is no such widget provided. Are you logged in as Admin under the exact same username account that registered the blog? If you are then this support doc is for you https://en.support.wordpress.com/writing-helper/request-feedback/

  I am having the same issues and your linked article does call it a widget. Please help!
