required form field is pre filling with my own email

Not sure why after i create a form, and preview and then publish, the required field for name and email is populated with my own. I have trashed the page and rebuilt it and the form, but the same problem.
Where is it finding this info and how can I prevent that ?The blog I need help with is: (visible only to logged in users)

There will be auto-filled information in a contact form or a comment box but your information is visible to you when you are logged in and have not cleared your browser cache and cookies. No one else can see it.
When someone makes a comment on your blog, unless they have cleared their cookies, WordPress.com will recognize them and complete the details for them. No one else will be able to view those details in comment boxes or in contact forms. The next person who comments will see blank fields. Commenters will not see each others details.
There is also an Admin [EDIT] link on posts and pages that only you as Admin and any Editors you have created can see when you are logged in. It’s for your convenience and your visitors cannot see it. You cannot remove it as it’s coded into the theme templates and we cannot edit templates or themes.

Note: If you log out, clear your browser cache https://en.support.wordpress.com/browser-issues/#clearing-your-browser-cache and cookies https://en.support.wordpress.com/browser-issues/#clearing-wordpress-com-cookies, and visit then your blog without logging in you can prove what I said above to yourself.
