I had a WordPress.com site a few years ago for a travel blog I did while living in China. I paid for the premium plan, so I had my own domain name and a premium theme. I stopped using the site when I returned to the U.S. and let the domain name lapse, but now I want to revive and redesign the site. I can still access the site through a WordPress address (xxx.wordpress.com). I want to get a new domain name and a new theme, and make sure all my old posts are transferred to the new-look site. Can someone tell me how to do this?

Your content is hosted here and can stay where it is. You can purchase the domain after selecting a premium plan, and separately purchase a premium theme https://theme.wordpress.com/themes/sort/premium/ for the site.

Details:
There are no trial upgrades, no monthly payment plans, no bulk deal upgrades, and no multiple year upgrades.

Registering an underlying .wordpress.com URL first is required for domain mapping. What domain mapping does is providing a seamless redirect to the very same content under the new domain URL when a person clicks the old URL .wordpress.com to the original content. The Premium Plan does not include access to premium themes. After I pay for the premium plan and pick a new theme and new domain name, how do I organize all my old blog posts in the new format? In my case, I have hundreds of old posts about a year I spent in China. Now I’ll want to organize them all in one section called China. Also, will all my previous followers be automatically notified of the new domain name and will all the old data about activity on my site be transferred over?

Welcome back :)
Your content is going to remain the same when you add a different them to the site. The design is all that changes when you do this. After you add a custom domain and set it as the primary that is the domain your visitors will see in their browser, but the behind the scenes site at the .wordpress.com address remains the same. The content and design will cary over to it.
Now I’ll want to organize them all in one section called China.
You’ll use Categories to do this.
