Search Box Widget Frozen/Broken

    I experienced a loss of my theme formatting yesterday . It is Koi.
    It was restored after some time but the search box in the sidebar is not working. It is frozen/broken but I cannot use it or remove it from the appearance sidebar. I have added a new search widget to the sidebar and it is working. (footer #2) but the frozen one (footer #3) is still visible and inactive.

    The blog I need help with is: (visible only to logged in users)

    removed search widget from Header (it had stopped working)
    added new search widget in the side bar.
