Sign up & comment problem

My sign up and comment fields are appearing empty for outside users although seemed ok when I tested.
The blog I need help with is: (visible only to logged in users)

I was able to post a comment (already logged in to my own WP.com account). I also could while logged out (prompted for email address). “Register” wanted me to register with wordpress.com – what was your goal?

For some reason having a photo on the contacts page – or at least how I had the photo? – seemed to prevent people from using the contact form. I’ve redone the page without the photo and now functions – at least I got a test to go through. Thank you for helping me resolve!
