Sign-up Sheet Plug-in

Our site, and probably many other non-profit sites, could really use a plug-in to create sign-up sheets. I’ve tried several of the suggested options – google, Event Brite, etc – but none are simple enough to get the members (teens) to reliably use. I just need a simple chart that could be put on a page for the event and then members could enter their name and a Yes or No for attending. An additional column for paid status, what your bringing, comments, etc. would be icing on the cake! It’s important that the entered information remain visible on the page for other members to see. There is no FTP access and no plugin capability on free hosted WordPress.com blogs.
