    Firstly; am I able to change the blog title, URL address and design of my site without losing content, comments and followers?

    Secondly; I’d like to switch to a premium account and get a better branded and designed site. Am I able to pay extra to have the site designed for me, in line with my requirements?

    Your blog title and tagline you can change easily at Settings ->General in the dashboard. It won’t affect your followers or content in any way.

    If you buy the premium package it includes a custom domain. That also won’t affect your followers as your WordPress.com blog will still be in the same place, and your permalinks are automatically updated. Your custom domain gets mapped to the WordPress.com base site. http://en.support.wordpress.com/domains/

    The premium bundle includes the custom design upgrade which allows you to make changes to your theme.
